Imported feed and feed additives continuation application documents

Required to submit the following application materials (in Chinese and English, all original documents and one photocopy, original of which shall be provided in English issued by the original manufacturer) and product samples:

1. "Imported feed and feed additive registration renewal application form."

2 of the original registration certificate (copy)

3. The quality standard materials

4 products in the latest specification

5-producing countries (regions) the latest approved (copy)

6 manufacturer registration issued by the commission to the agent power of attorney

7 For a failure of supervision and random inspection of imported feed and feed additives, to be submitted three batches of product samples, product quality inspection and review.

8. Other documents required by the Ministry of Agriculture (if any)
